The Initial Intake Form collects basic information about you and your child, including contact information and information about your child’s communication needs, so that we can respond to your inquiry and prepare for an initial consultation.
The Initial Intake Form is provided through Google Forms and uses Google Workspace. Information submitted through the form may be processed and stored by Google in accordance with Google’s applicable privacy policies, terms, and HIPAA-related agreements. Dot & Dash uses Google Workspace under its applicable Business Associate Agreement with Google.
Please do not submit detailed medical or other sensitive health information through the Initial Intake Form. Additional health and medical information will be collected through the appropriate patient forms if you and your child move forward with services.
Email may be used for clinical communication when appropriate. Text is best for quick questions and scheduling; please do not send private health or clinical details by text.
